- Replaced generic NY State of Health links with the direct cost-sharing page for diabetes and pregnancy tips.
- Replaced the broad Medicaid home/community page with the direct environmental-modification guidelines.
- Replaced the old NHTD manual link with the current status page and corrected the action because new NHTD referrals are paused.
- Replaced the missing/indirect Big Apple Connect page with the current ACCESS NYC program page.
