Target Demographics : Pregnant women, breastfeeding mothers (up to 1 year postpartum), non-breastfeeding postpartum mothers (up to 6 months), infants, and children up to age 5 Parents, grandparents, foster parents, or legal guardians can apply for their eligible children Residency : Applicants must reside in Nassau County, NY.
What to bring
Documents : Identification for each applicant, recent 30-day proof of income, proof of physical address, and immunization records for children.
