Eligibility
Generally age 60+ and homebound, unable to shop or cook because of illness, frailty or disability, and without adequate help; spouses may also qualify.
What to bring
No documents are needed for pickup; ask the case manager what information is required for assessment.
Requirements
An in-home assessment by a case manager is required.
