Eligibility: The person must be waiver-eligible, the transition must be from a nursing home to the community and expenses must be approved through the service plan/RRDC process.
What you get
CTS can cover approved one-time transition expenses such as moving, security deposits, broker fees, essential furnishings, utility setup/deposits and certain pre-occupancy health and safety costs.
